Root Cause Failure Analysis (RCFA) is a deductive technique of identifying causes that lead to failure, or failure recurrence. Causes are identified based on logical cause and effect relationships by answering to the question of “Why did this happen?”, or “What can cause this?”
The goal of RCFA is reliability improvement. Reliability is the ability of a system or component to perform its designed function under stated conditions throughout its life cycle. The Root Cause Analysis Solutions (RCAS) technique is designed to support the RCFA goals and is oriented toward problem solving and solution building.
